Patient-Requested Forms (FMLA, Disability)

WashU Medicine Orthopedics charges a $15 fee to process patient forms, such as FMLA or disability forms. We must receive payment before we can return your completed forms.

Please follow these steps:

Pay the $15 fee using cash, check or credit card. Payments can be made in person at the front desk of any of our locations, or by calling 314-514-3500 to pay over the phone with a credit card.
Fill out and sign your part of the form
Allow up to 10 days for us to finish your paperwork
