Commitment to Excellence in Education

In conjunction with the Department's mission to actively involve fellows in the educational process of resident physicians in training, hand fellows are called upon and are responsible for presentations at departmental educational conferences. These include, but are not limited to presentations at Grand Rounds, Distinguished Lecture events and the Arthur J Stein Memorial Lecture which is hosted by the Upper Extremity Services in the Spring of each year.

The Arthur J. Stein Memorial Lecture Series

This lecture series is hosted by the Upper Extremity Service in the Department of Orthopaedic Surgery and is typically held on a Wednesday in the Spring of each year.

The lecture series provides the fellows, residents and faculty with the opportunity to learn more about advanced topics in Orthopaedic Surgery from internationally distinguished surgeons. The day is comprised of both formal and informal educational forums.

The Outpatient Experience

The fellow functions in two types of outpatient clinic settings - private practice and hospital-based clinics.

Private Practice
Fellows see and evaluate all "new", postoperative and follow-up patients with non-operative upper extremity musculoskeletal problems in the private practice outpatient clinics of the eight attending surgeons.

Presentations include history and examination, diagnosis, and proposed treatment plan. In this setting, the fellow is expected to participate in the education of the residents and fourth year medical students who are also present in the private outpatient clinic.

Hospital-based Clinics
Barnes-Jewish Hospital has one adult hand clinics; in this setting, the fellows are the patients' primary physicians for their hand-related conditions. Patients seen in the hospital-based clinics are reviewed with the attending physician, particularly those who are candidates for a surgery.

In Patient Care

The fellow participates in the care of all hand surgery inpatients on the hospital wards. Under the supervision and guidance of the attending surgeon, the fellow is responsible for the coordination of preoperative testing, treatment and postoperative decisions. The fellow also teaches and supervises the junior and senior level residents assigned to the Hand and Upper Extremity Service.

The Operative Experience

Responsibilities include:

    • Assisting the attend hand surgeon at surgery
    • Performing surgery under the direct supervision of the faculty attending surgeon
    • Performing surgery independently after discussion of case with and approval from the faculty attending

The fellows are allowed increasing levels of responsibility commensurate with experience, competence and technical skills.

On Call Assignments and Responsibilities

    • The Department of Orthopaedic Surgery and the Division of Plastic Surgery alternate Emergency Room Call Coverage at Barnes-Jewish Hospital and Saint Louis Children's Hospital from home on an every-other-night basis. With two Hand Fellows from Orthopaedics and two from Plastics, an Orthopaedic fellow can expect to cover call from home one out of four evenings in this capacity

    • The Orthopaedic Surgery Hand Fellows share the responsibility of answering after-hours calls from patients of the Orthopaedic faculty attendings. Call of this nature is also taken from home by the Fellows. Fellows alternate weekday and weekend call coverage.

Anatomy Conference

Fellows read and discuss articles with resident and attending staff from Journal of Hand Surgery and appropriate articles on the Upper Extremity from the Journal of Bone and Joint Surgery.

This conference series has its own fully developed syllabus with relevant readings and also includes dissection of fresh cadavers.

Tuesday am CORE Conference Based on CORE Curriculum designed for the Orthopaedic Residency Program. Fellows' participation in these conferences is optional, although fellows are encouraged to attend all lectures relevant to the Upper Extremity.
Wednesday Grand Rounds Hand or Upper Extremity Presentations given once every few months by a Hand Fellow or Chief Resident on the Hand Service. Presentations are supervised by a faculty member. Attendance at other Grand Rounds Presentations is optional.
Thursday Interactive Conferences Hand/Upper Extremity Interactive Conferences are held every fourth Thursday. Fellows present short cases (usually a patient recently treated by the service) to the resident body.

Rotation or Sample Daily Schedule

The structure of the fellowship currently is comprised of four separate rotations, each three months in duration.

The fellows' weekly schedules vary, depending on the service on which they are rotating.

The fellows rotate in both clinic and the OR with the attending surgeons with whom they are working. A good balance between outpatient clinical and operative patient exposure is achieved.

On August 1st, each Hand Fellow is assigned to his/her rotations. Fellows rotate with a given team for three months at a time.
